Required Registration Documents

The following items must be submitted to complete your athlete's registration for the 2026 season:

☐ Participant Tracker Form (Page 2)

☐ Printed copy of your child’s 2025–2026 school year report card

☐ Physical Examination Form dated January 1, 2026 or later
OR
☐ Medical Clearance Form completed, signed, dated, and STAMPED  by your physician 

☐ Original Birth Certificate with Seal (New Participants Only)
Please note: We are required to keep the original birth certificate until it has been reviewed by the AYF League. It will be returned afterward.

Physical Requirement

All participants must have a current physical dated January 1, 2026 or later to be eligible to participate in the 2026 season.

If your child’s annual physical is scheduled after July 31, 2026, you may submit a completed Medical Clearance Form (signed, dated, and stamped by your physician) before July 31, 2026. We will not accept a physical form without the office stamp. 

Deadline

All forms and required paperwork must be submitted by July 29, 2026, in order for your athlete to participate in the 2026 season.
